To determine student eligibility for a 504 Accomodation Plan:

Does the student have a physical or mental impairment? Does that impairment substantially limit a major life activity?

Does the student require reasonable accomodations in the classroom or around the school?

If a student needs accomodations then a 504 plan should be created with the appropriate team. Teams may include: 504 coordinator, teacher, parent/guardian, nurse, etc.
